Medical Massage Requirements

Insurance accepted: First choice, Aetna, MVA (pip),

NO I DO NOT TAKE L&I, REGENCE, PREMERA OR BC/BS or United healthcare due to non payment from them

all must have a referral or a prescription which must be faxed prior to the appointment.  If your insurance says they don't need one, that's great I still do.  By law I can not bill without diagnosis codes which can only be given by a Dr. or Chiro. 

The Client intake forms must be COMPLETELY FILLED OUT PRIOR to the appointment.  The forms will be emailed to you upon scheduling the appointment.  The forms contain information needed at the first appointment and the time scheduled does NOT allow for filling out of paperwork.  

Unfortunately too many people have come to the appointment without the paperwork completed and I have had to make adjustments for this at my expense so there are NO EXCEPTIONS.
